Keep your skin looking healthy and vibrant. Get help for acne, eczema, rosacea, poison ivy/oak, and psoriasis. Commonly prescribed medications: Hydrocortisone, Triamcinolone, Hydroxyzine, Metronidazole cream, Clindamycin lotion Provider may prescribe medications that are also available over-the-counter. OTC medications may not be covered by insurance, and getting medication through a prescription may be cheaper. Insurance coverage and pricing vary by plan. Typical number of refills if prescribed: 1 pack, 0 refills
Cancellation Policy
Client Financial Responsibility Thank you for choosing First Priority Clinic PLLC for care. We ask that you read this form to acknowledge and agree to accept financial responsibility for services rendered by Provider to Client. I agree that I am legally responsible and agree to pay to the Provider for all fees, charges, and expenses incurred by the Client or owed to the aforementioned practice in connection to Provider providing care to Client. I acknowledge and agree that I am ultimately responsible for the payment to Provider for any and all services rendered by Provider to Client. No-Show/Cancellations No-shows and cancellations will be charged a $25 fee.
