Service Description
We are here when you need us. Commonly prescribed medications: Ella (ulipristal) or Plan B (levonorgestrel) Plan B and Ella are both safe and effective forms of emergency contraception that can reduce the risk of unintended pregnancy. Both Plan B and Ella are more effective the sooner you take it. Ella is only available via prescription and can be taken up to 5 days after unprotected sex. Ella is more effective than Plan B if your BMI is 30 or more. Ella is not effective if your BMI is 35 or more. Here is a link to calculate your BMI to determine if Ella may be right for you. Plan B is available over the counter. It can be taken up to 3 days after unprotected sex. Plan B is most effective if your BMI is less than 25, and it is less effective if you BMI is 30 or greater. Typical number of refills if prescribed: Initial: 1 pack, 0 refills

Client Financial Responsibility Thank you for choosing First Priority Clinic PLLC for care. We ask that you read this form to acknowledge and agree to accept financial responsibility for services rendered by Provider to Client. I agree that I am legally responsible and agree to pay to the Provider for all fees, charges, and expenses incurred by the Client or owed to the aforementioned practice in connection to Provider providing care to Client. I acknowledge and agree that I am ultimately responsible for the payment to Provider for any and all services rendered by Provider to Client. No-Show/Cancellations No-shows and cancellations will be charged a $25 fee.
