Need a fast prescription refill? We can help. Commonly prescribed medications: Diuretics (hydrochlorothiazide, chlorthalidone), ACE inhibitors (lisinopril), Beta Blockers (metoprolol, carvedilol), ARBs (Losartan), & Calcium channel blockers (amlodipine), including combination pills Typical number of refills if prescribed: 90-day supply with 0 refills per year if the provider determines it is clinically appropriate for you Patients may not receive more than one prescription every 12 months through this service. If you need another refill within 12 months, please follow up with your primary care provider
Cancellation Policy
Client Financial Responsibility Thank you for choosing First Priority Clinic PLLC for care. We ask that you read this form to acknowledge and agree to accept financial responsibility for services rendered by Provider to Client. I agree that I am legally responsible and agree to pay to the Provider for all fees, charges, and expenses incurred by the Client or owed to the aforementioned practice in connection to Provider providing care to Client. I acknowledge and agree that I am ultimately responsible for the payment to Provider for any and all services rendered by Provider to Client. No-Show/Cancellations No-shows and cancellations will be charged a $25 fee.
